My Tools 4 Care - In Care
This is a toolkit to help you as a care partner prepare for possible changes or events, so you can support your family member with dementia in care. “In care” means receiving 24-hour care in a facility, for example, long-term care. It is based on a research study and what other carers have told us. This toolkit can be used as you wish, by you and those close to you. No one else will see it unless you share it with them.
My Tools 4 Care - In Care is divided into 4 sections: 1) About Me (activities for you to engage in); 2) Common Changes to Expect; 3) Frequently Asked Questions; 4) Resources. You do not have to go through the sections in order. You can choose to use different sections of this toolkit depending on what you feel you need. There is no need to complete all of the activities if you don’t want to. It is up to you.
